Team Triathlon Entry Form

Use this form to submit the details of your Triathlon Team entries.

Triathlon Substitution Form

Use this form for substitutions to your Triathlon Teams.

You will need to complete a new/separate form for each of your Triathlon Teams  and / or different Triathlon classes. 

However you can put up to 3 substitutions in for the same team and class on one form.

Grievance & Disputes Form

Use this form to submit any grievances or disputes during the championships.
